What Insurances does PAPT Accept:

Please check your insurance company below to see what you need to receive physical therapy services at PAPT. New York State requires that any patient being seen for physical therapy care have a prescription. Whatever your insurance company, it would be helpful to your therapist if you brought any results of tests that you have had related to your injury such as MRI's, X-rays and if you had surgery your doctors post-operative report and protocols.

Aetna

You will need a referral from your Primary Care physician to receive physical therapy as well as a prescription as mandated  by New York state law.

Oxford

You will need a referral from your Primary Care physician to receive physical therapy as well as a prescription as mandated  by New York state law.

Cigna:

 If you are in the PPO plan you will need a prescription from your doctor for physical therapy services.

If you are in the open Access Plan, please call the office and speak with Charu, our insurance liaison, to get details for receiving physical therapy services.

Blue Cross/ Blue Sheild:

You need to bring your insurance card and a Prescription from your Doctor.

SAG (Screen Actors Guild):

You need to bring your insurance card and a prescription from your doctor.

Health Net

You need to bring your insurance card and a prescription from your doctor.

Great West

You need to bring your insurance card and a prescription from your doctor.

United Health Care

Please call the office and speak with Charu, our insurance liaison, to get details for receiving physical therapy services.

Health Plus

You will need a referral from your Primary Care Physician to receive physical therapy as well as a prescription

Medicare

You need to bring your insurance card and a prescription from your doctor.

PAPT Accepts Workers Compensation Insurance from these companies - Liberty Mutual, Chubb, AIG, St. Paul, State Insurance Fund, Injured Workers Insurance Fund, The Hartford Insurance Company.

Please Note: All Workers Compensation patients need a C2 form from their employer and a prescription from their doctor.

We also will provide therapy out of network for most other insurance plans. Please call for further information to utilize PAPT as your provider for rehabilitation services. 212-245-PAPT (7278) ask for Charu.
