Paper Work Do I Need To Recieve Physical Therapy Services?
There are a number of forms that need to be filled out before you can recieve physical therapy services at PAPT.
You can either come early to your appointment on your first vist to fill them out or download them here and bring them with you already filled out
to avoid taking the time of your evaluation to fill out paper work. Our patient medical and personal history provides information to the therapist so
that they can better evaluate your particular needs based on your medical history and personal injury history as well as you work history.
Due to the government requiring all medical facilities to safe guard all patient's personal and medical information there are 3 other forms that need to be filled out
by you to ensure that we have met the governmental requirements in safe guarding your information.
